One filing describes driving on a motorway with small children in the back when 'a loud pop sounded and glass from the moon roof came shattering down upon us', hitting the driver's face, arms and hair; she was able to exit safely and the children were not struck. Similar reports appear on the neighbouring model years, including a rear windscreen that shattered in a driveway with a security camera confirming nobody was near the van. There is no recall and no Toyota bulletin for this. Practically: check the roof and rear glass for chips and stress marks before you buy, and if you are buying used, ask whether either has been replaced and why. Comprehensive insurance covers glass; the shock in a van full of children is the real cost.
